Choir Forms
This form is to be used when a student has a conflict and knows about it at least 24 hrs in advance. Fill this form out and hand it to the choir directors. They will approve or not approve the absence at this time.
This form is to be used when the student has all ready missed a rehearsal. Once it has been signed, the directors will approve or not approve the absence.

This form is to be used when a student is leaving an event with someone that is not their guardian.
